It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.

In the event of illness, injury, and/or accident, I authorize the camp instructor or any Field Day employee to act on my behalf. They may approve any and all non-emergency or emergency treatment and are authorized to sign any and all medical releases or required form(s) on my behalf. In the event of an emergency, I understand that I will be notified of the situation as soon as possible. I understand that I am responsible for all expenses in the medical treatment of my child, including, but not limited to all transportation costs to and from a medical facility, and, if necessary, transportation to my home or medical facility of choice.

My Signature below waives and/or releases Field Day from any and all liability and/or financial responsibility for any medical expenses incurred.

I understand that Field Day, in its sole discretion, may dismiss any camp participant for inappropriate, disrespectful, or dangerous behavior at any time. In this event, I understand that I will not receive a refund of camp fees for unattended days. If my child breaks or damages any property as a result of their direct or indirect behavior, I hereby agree to pay for its repair or replacement.

I hereby certify that my child has been cleared by a physician within the last calendar year to participate in strenuous sports activities (see registration form above). I understand that the risks associated with camp activities could result in injury and/or death to my child. I hereby assume these risks and, knowing them, hereby give my child permission to participate. I understand that the Field Day is not liable for any injuries or other occurrences due to indoor and outdoor camp activities or related risks, and/or the actions or omissions of Field Day Sports Summer Camp counselors, volunteers, employees, directors, officers, or any other entities being released.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which my child may participate and that it will govern the actions and responsibilities at said activity.

In consideration of my application and permitting my child to participate in Field Day Sports Summer Camp, I hereby:

WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of Field Day, officers, employees, camp counselors, volunteers, or other persons, for my child’s death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to them including their traveling to and from this activity;

INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE Field Day, officers, employees, volunteers, or other entities or persons released from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

I consent and agree that Field Day and/or their staff members, contractors, coaches, agents, representatives, or volunteers may take photographs or digital recordings of my child as a participant during this event and use these in any and all media for promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims, or interest and I understand that there will be no financial or other remuneration.

Refund Policy
All camp registration fees are NON-REFUNDABLE. By completing the registration process and submitting payment, you acknowledge and agree that NO refunds will be issued under any circumstances.

Each registration incurs administrative and operational costs, and payments are used to secure camp resources, staffing, and materials. Because of these expenses, we cannot offer refunds once a registration is processed.

No refunds or credits will be given for: a change of mind after registration, scheduling conflicts or other personal reasons, failure to attend camp for any reason, campers arriving late, campers leaving early, campers dismissed from camp, or campers unable to attend all or part of a session.

By registering, you confirm that you have read and agreed to this refund policy.

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. The Field Day, officers, and all its employees acting officially or otherwise are hereby released from any and all claims, demands, actions, or causes of action on account of any injury to my child that may occur. This release binds my heirs, executors, administrators, and/or assigns.
